Since the program began in 2004, this dynamic team of clinicians has supported hundreds of aging out youth in their drive for independence by coupling the following:
- Daily Living Skills training,
- Client centered services,
- Community-based delivery,
- Intensive case management,
- Mental health services, and
- Primary care linkages.
Due to the lack of lifetime supports, the aging out population is at much higher risk of unemployment, incarceration, and poverty than their peers.
Adult and Child provides both Transitional Foster Care Homes and Scattered Site Apartments and follow curriculum developed by Casey Family Programs designed to capitalize on teachable moments in each person s life and to coach consumers through real life challenges. Furthermore, services are designed to fit individual needs; for example, there are a considerable number of teen parents involved in the transitional living program. To support them, Adult and Child now provides parenting education and support services to the teen parents involved in transitional living.
Housing and Basic Needs After a quick assessment of needs is completed, each case manager will immediately begin teaching each youth to identify and secure:
- Appropriate housing,
- Sources of healthy food
- Access to healthcare resources.
Adult and Child case managers are very experienced in community coordination and have strong relationships with local housing owners.
Educational & Vocational Training High School graduation or GED is an important indicator of gainful employment. Many national and local level jobs require a minimum of GED training.
- Encourage each youth to achieve their educational goals.
- Coordinate with local schools and GED programs.
- Offer specialized GED programming.
- Coordinate vocational training with employers and with supported employment program.
Emotional and social support is critical to the long term success of transitioning youth to independence.
- When appropriate, each person is linked with counseling and psychiatric support.
- Each youth is encouraged and supported in building appropriate social support networks through family, friends, neighbors, and churches.
- Each person`s level of functioning is continuously assessed and monitored throughout the transition planning process.
