At Adult and Child, we are proud of our customer service reputation.  If you would like to file a complaint or report a concern of any type, we encourage you to do so.  Here is how.

Reports can be made by:

Contacting the Director of Quality Improvement and Corporate Compliance Officer, Dr. Pat Novak at 893-0285.

Filling in any of the forms below and sending to Dr. Novak in an envelope that is provided.

Call the CONFIDENTIAL HOT LINE at 893-0201.

Completing a Corporate Compliance Report Form which may also be found on the Adult and Child intranet.

If you have concerns about the safety or quality of care of a client, you may report this to The Joint Commission at (800)994-6610.

Adult and Child will take no disciplinary action for reporting concerns regarding client safety or quality of care and quality of care will not be affected by a consumer`s reports of a concern or grievance.
